Woke up last few days, shaky, weak, tired, headachey, a bit dizzy (near nauseated), so still battling, and haven't been doing good last two days, and in right shoulder, and shoulder blade area pain. Got packet in mail for BlueCross, chose Sutter, but it will be a few days before they get back to me (two weeks), before I can even make an appointment.

Nausea is worse now, much worse. Very weak.

Laboured breathing, feel like throwing up.

I am getting 'ear clicking' now, and I thought it was just something in the room I was picking up, but after looking it up, seems to deal with the inner ear and eustachian tubes, and I feel fluid build up, and like head like under water, or pressure build up, echoing in ear when I breathe in and out.

No blood in stool that is seen, and nothing was immediately seen on CT or ultrasound of abdomen last few ER visits. I do not smoke, never smoked, do not drink alcohol, or use 'drugs', and even until recently I hadn't used prescribed, or over the counter 'meds' since 2008. It began in 2024 when I was ER hospitalized in American Samoa, and nearly died.

All the symptoms are from PCS - Postcholecystectomy syndrome - Wikipedia

I have all of those things (except vomiting, though came close a few times, as the nausea is extreme and the worst part, beside the bloating of intestines which makes it hurt to breath (laboured breathing) and causes my right arm injury to be in constant increased pain, down whole right side, shoulder blade) post surgery on Gall Bladder (late 2025, Nov.).

You can also visit/consult professional nutrition specialists. Ideally in some clinic.

You will probably need to dramatically change your diet, if doctors cannot find anything. Bloating, spasms, pains etc. are from wrong foods, toxins, messed up microbiome and so forth. But it can also be strongly linked to you mental problems, so a psychiatrist can also help.

The longer this will continue, the more you will fall apart and more allergies, dyspepsia, leaky gut, sensitivities and mental problems you will develop and so less and less foods will be suitable for you and you will have to end up on something like the lion diet for life, from necessity (regardless if you like the idea or not).

yes, understand (i need to be officially diagnosed, tested, waiting on paperwork to have PCP, primary care physician, and may need another ER visit before, but have been reluctant to go, since how I have been 'treated' so far), but doing what i can to read on my symptoms, conditions, as i am able, to at least know what i might ask a doctor about if i can see one again, since i have no immediate help, last ER visit (kaiser) i was essentially thrown out and they wouldn't listen or help me, and I am in constant pain (+7/10), constant extreme nausea, and now sleep is fitful, maybe 4 hours because of this

i copied (too exhausted to type it all) this from my list of symptoms; which are at least 3 things (right foot / leg issue from imbalance being exacerbated, right shoulder injury being exacerbated, and post-surgical gall-bladder removal complications (nausea, bloating, increased pain, alternations of constipation / diarrhea, &c):

Internal Right Shoulder Injury (not broken bones)

Right foot (right side) feels off balance (like walking on right, not landing straight, twisted inward), Right Knee (extreme sharp pain), Right hip pains (right hip clicking / popping) when walking for 30 minutes

Symptoms (Chronic / Daily / hourly, for last 4 months at least for intestinal, last 7-8 years for right arm, last 5 years for right foot / leg / hip):

- Extreme Abdominal / Intestinal Bloating / Swelling (for hours 8+)

- Abdominal physical distension (painful), like pregnant

- Continual Flatulence (especially in morning, when laying down on sides (left or right), but when standing doesn't move as much, even when walking

- Inflammed Bowels / Intestines, Irritated (painful, like on fire, sometimes a fiery feeling in lower left bowel near left hip, or all over / general)

- EXTREME Chronic Nausea / dizzyness (like going to constantly throw up, but no vomit, though came close a few times). for over 8+ hours a day

- Shaking / spasmodic muscles (all over, like cold, shivering), uncontrollable muscle spasms / shaking

- Extreme weakness / tiredness (day long)

- Rapid burst Heart rate changes (extreme fast / rapid to sudden slow, like no energy, can physically feel the 'drop'), not all the time, but on occasion, usually when laying down

- Sometimes feels like blood pressure drops suddenly, like no immediate energy, a 'blah' feeling, or out of breath feeling, not continually, tied with heart rate changes

- Hot / Cold flashes, accompanied with sweating, all over (rapid, quick, not prolonged, like a flu like symptom, or toxicity)

- Muscle tension, and Head tension (extreme, like a vice squeezing constantly, in back and middle / center), back neck tension (pain)

- Headaches accompanied with back neck into middle head pain, like back neck is on fire, fire crawling up neck into head

- Head 'pinging', like there is an ongoing high pitch whine, a "reeee", "shing", "beeeeeeep" mostly in back left head behind back left ear (not in ear directly), like electrical signal constantly sounding steady a flat-line noise

- Right ear internal eustachian tube "clicking", like beetle clicking, accompanied with head pressure, feels like fluid in head (both sides), like drowning

- Right side body extreme pain in right side ribcage (middle, down whole side, front, side and back, with right side muscle pains especially in right side neck / shoulder, middle right shoulder (internal), and shoulder blade area EXTREME pains (+7 or higher nearly at all times)

- Right shoulder internal EXTREME pain at all times (+7 or higher, for last 7-8 years) up into neck, down right side, like I have been punched, weakness in right arm, cannot sustain lifting, raised potion for more than a minute of two at max, not broken bones

- Laboured breathing, when abdominal is bloated, like can't catch breath, or not enough oxygen, forcing vocal noises to breath ('huh', 'huh')

- Alternating between constipation & diarrhea

- Slow (days long, 2-3 days sometimes) bowel motility

- sometimes yellow bowel liquid in toilet (not at all times)

- Whole body nerves feel like they are on fire, or irritated, cannot get comfortable, touching things makes uncomfortable, even wearing clothes, like I want them to stop touching me

- sleepiness / tired / exhaustion, but cannot rest / sleep well, tossing, turning, fitful, nerves 'pinging'; continual yawning throughout the day, sometimes like near to passing out / collapsing

- sometimes burning acid in throat (rare, not always)

- before surgery had continual weight gain, as gall bladder was not functioning properly to break down fats, now lots of weight loss

- before and after surgey, still have acne showing up on upper arms and upper thighs that I did not have before a few months ago, as body is dumping stuff through skin

- irritability, some food anxiety (like in eating because I do not want to feel sick any more after eating, and feel sick if I do not eat)

- depression / sadness because of pain, &c.

Medication 1

Things I have learned living without a gallbladder and living with my intestines pushed together because of major kyphosis , my spine is very bent .

Alternating between constipation & diarrhea

- Slow (days long, 2-3 days sometimes) bowel motility
I did not see any fiber on your list , Metamucil or equivalent will do , psyllium husk is the ingredient you need
Fiber will regulate your digestion to make it normal no more back and forth as you have .
I just take one dose a day now but if I have trouble will take more .
Probiotics (Medication Name) - Health Aid Daily Probio (10 billion CFU with Prebiotic)
How many a day you taking of these ? I like to take at least a 30 billion and up . I have good results with L Reuteri probiotics too .


What is typical day of food intake ? What are you eating ?

If your extreme bloating is when you are constipated an enema is what I have found to be effective relief .

I eat a plant-based diet (fruits & veggies), and so lots of fiber.

No, I do not have metamucil / psyllium husk yet, may have to get some and add it to the list. Just been so exhausted, nauseated, hard to go anywhere, may ask my family to pick it up for me.

Meals have been sporadic lately (but trying to eat for nutrients), due to food anxiety / fear and not wanting to have more nausea, pain, bloating, but, have had fruit smoothie (various berries, banana, tangerine, pineapple (for bromelain), sometimes with ground flaxseed mixed in, &c.), in morning, and a meal at night (small, like steamed carrots (table spoonful) last night, vege-potato salad (table spoonful), half a left-over vege-burger (w/ lettuce, tomato, avocado) on soughdough toasted bread), with various teas, water, throughout the day, as I try to keep separated major fruits / veggies (for mixing them causes fermentational gas, which I am trying to avoid, unless it is like a tomoatoe or something like banana that can mix ok), and most dinner meals are something like that, nothing over complicated, and pretty simple, steamed / baked squash, steamed cauliflower / broccoli, maybe a homemade vege taco (flour tortilla, and veggies, maybe with cooked firm tofu), no hot sauces or anything like that, usually just use lime (fresh or bottled). Sometimes it is like Kellogs raisin bran cereal (small half bowl) with almond milk, or plain steelcut oats oatmeal with cut apple slices (and I avoid things that irritate gut lining like cinnamon). I have also tried gluten free for a time, didn't seem to help, and made things more constipated. Not always, sometimes I cannot eat in morning / afternoon due to nausea, and just have tea, or water.

Probiotics, some in morning, some at night, before meal.

I have considered 'enema', but only as a last resort, as prune juice (1 small juice glass) generally flushes me between 30 minutes to 2-3 hours.

Your post below is when should try it , usually my problem besides what you mentioned below I will have lots of belching too .
Please, I am in such misery today, same extreme nauseuousness, bloated, distended abdomen, hot/cold flashes, uncomfortable all over,

- **Bloating and Indigestion:** Because bile cannot be released in a large quantity instantly during meals to help digest high-fat foods, some patients may experience bloating, belching, or mild discomfort in the upper right abdomen when eating greasy food in the early post-operative period. This usually requires the body to readapt, which can be alleviated by eating smaller, more frequent meals and chewing food thoroughly.
- **Bile Reflux Gastritis:** Due to the continuous flow of bile into the duodenum, if the pylorus (the valve between the stomach and duodenum) doesn't close properly, bile can reflux back into the stomach. This can cause symptoms like stomach pain, heartburn, and a bitter taste in the mouth.

### 3. Compensatory Dilation of the Common Bile Duct and Stone Recurrence
- **Compensatory Dilation:** After cholecystectomy, the common bile duct partially takes over the gallbladder's function and withstands greater pressure. Therefore, the common bile duct may dilate slightly. This is a normal physiological compensatory phenomenon.
- **Common Bile Duct Stones:** Although the gallbladder is gone, **bile is still produced by the liver**. If the bile composition is too viscous (high in cholesterol), or if drainage from the common bile duct is poor, bile can re-congeal within the common bile duct itself, forming new stones. This is called "primary common bile duct stones" and is unrelated to the gallbladder. Therefore, maintaining healthy eating habits after surgery is still essential to prevent stone recurrence.

### 4. Rare Long-Term Issues Related to Surgery
- **Sphincter of Oddi Dysfunction:** The "valve" where the common bile duct enters the intestine is called the Sphincter of Oddi. Its function can become disordered after surgery, potentially leading to spasms that cause abdominal pain, fever, or jaundice.
- **Bile Duct Injury (Intraoperative Complication):** This is a serious complication that can occur during the surgery itself but usually manifests shortly after the operation with symptoms like jaundice or abdominal pain, and is not considered a long-term sequela.

### 5. Controversy Regarding Colon Cancer Risk
Some older epidemiological studies suggested that after cholecystectomy, due to increased stimulation of the colon by secondary bile acids, the risk of colon cancer might be slightly elevated. However, recent research is controversial, and any potential risk increase is very minimal. For most patients undergoing surgery for gallstones, the inflammation and pain caused by chronic cholecystitis are far more urgent to address than this minimal theoretical risk.

### Post-Operative Recommendations
1. **Dietary Adjustments:** For 1-3 months after surgery, strictly adhere to a low-fat diet, avoiding fried foods, fatty meats, and rich soups. Afterwards, you can gradually reintroduce foods to find your individual tolerance level.
2. **Eat Small, Frequent Meals:** This reduces the digestive system's burden with each meal and allows for more even utilization of bile.
3. **Increase Dietary Fiber:** Soluble fiber can help bind excess bile in the intestines, reducing diarrhea.
4. **Regular Check-ups:** It is advisable to have an annual abdominal ultrasound after surgery, primarily to monitor the common bile duct for any dilation or stones and to assess the liver's condition.

**Summary:**
Most people will return to a normal life after gallbladder removal; the body simply transitions from "releasing bile in a concentrated manner" to "dripping bile continuously." Diarrhea or indigestion may occur in the early stages, but through dietary adjustments, the body will gradually adapt to life without a gallbladder. What truly requires vigilance is not pain (as pain is usually caused by the inflammation or stones, which are removed), but the change in digestive patterns.
