It is. One is attacking Trump's policies, which is fair game when discussing politics. The other is attacking a person's character by saying they're deranged. It's the definition of ad hominem and it's something Trump does very often when he's losing the debate.

This is no different than the left calling someone a racist just because they have a difference of opinion. You're not actually engaging witg the argument. Instead you're going straight to ad hominem.
