I think you're looking at this with too broad a spectrum. As the harlot is within the context of the 70th week, so must the phrase, "all that were slain upon the earth" be considered within the context of the 70th week. One can not even blame a spiritual Babylon for all deaths from all time. If there is a "spiritual Babylon" it didn't come into being until some time after Noah and just like it took awhile for Christianity to spread across the earth, "spiritual Babylon" wasn't a global factor either. The only being who can be blamed for all deaths since the beginning of time would be satan. This verse is related to Rev. 17:6 and there hasn't always been saints and martyrs either. This verse is speaking of deaths within the period of the 70th week only, imo.
