That "altar" is a reference to the OT faithful of Israel.All of the redeemed are under the alter and in the Lamb's book of life.
The Atonement was sufficient for every single human ever conceived. Those who reject the Atonement will be removed from the Lamb's book of life.
Malachi 3:16. They were still waiting for The Promise to come, but had died in faith, before the Day of Pentecost. God did not forget them, and now their names are also written in the book of Life, being they who are shown in Revelation 6:9-11.
No one has been redeemed as of yet, except Jesus, who is the Firstborn from the dead, being Himself "the first resurrection", aka the First NEW creature.
All we, who are born again of/by His Spirit, are now made to be "partakers of His divine nature" (His resurrection), while we are still in our mortal flesh.
"Blessed and holy is he that HATH PART* in the first resurrection.....", who is Christ.
* Note: "hath part" is in the present tense.
