Here is a copy and paste from Bible hub of the NKJV translation...I have also changed the translation of the Greek word "Seismos" from "earthquake" to "turmoil" as it is a better fit to the context being referenced with this Greek word.
18And there were noises and thunderings and lightnings; and there was a great earthquake, such a mighty and great earthquake as had not occurred since men were on the earth. 19Now the great city was divided into three parts, and the cities of the nations fell.
Jay, don't be changing the text of the translation. The text in the translation as "earthquake" is correct.
The verse is talking about a global earthquake, greater in intensity and span than any previous time in man's history.
Babylon the great, in God's thoughts, represents man's rebellion against God. And the population of the earth with their mark of the beast, 666, alliance to the beast, and the kings of the earth assembling their armies to make war on Jesus and His armies of heaven - will all be global rebellion against God.
So the great earthquake will be global, likely along present fault lines, and the mantle of the earth's surface split into three segments. Affecting islands and mountains around the world.
And great hailstone falling on men around the world, as God's wrath will be taken out on the world in rebellion against Him.
