The thorn was not physical.....eyesight,limp,etc.
Can you imagine with having to live with the memory of stoning Stephen and destroying the Church? We all have things we want to forget. I can't imagine how he felt. But thanks to God's grace we endure.

Paul said it was in the flesh so it in fact was a physical affliction from Satan, have you read Job and how he was given boils?
Job 2:7 So went Satan forth from the presence of the LORD, and smote Job with sore boils from the sole of his foot unto his crown.
2 Corinthians 12:6-8
6 For though I would desire to glory, I shall not be a fool; for I will say the truth: but now I forbear, lest any man should think of me above that which he seeth me to be, or that he heareth of me.
7 And lest I should be exalted above measure through the abundance of the revelations, there was given to me a thorn in the flesh, the messenger of Satan to buffet me, lest I should be exalted above measure.
8 For this thing I besought the Lord thrice, that it might depart from me.

A lot of things are called flesh,even living after the flesh. Evil thoughts are freshly. Memory can relate to flesh. But I will not argue scripture. You are free to think as you will.

What problem with eyes? Scripture says nothing about it.He had a problem with his eyes, but that was NOT what was his "thorn in the flesh" nor messengers of satan.
