I would love to see a categorized list of all these med errors and see what type of error they fall into.
Analysis like this can be massaged in many different ways. How many were intentional errors, or errors because while treating, things went south. How many drug interactions cuz patients gave wrong histories etc.etc.etc.
YOu offer no solutions only gripes.
If we followed your implications, we should ban bad fats, sodium in processed foods, chemical additives, autos while we are at it (over 50K deaths a year due ot errors)
And I want to see your proof that pharmaceutical houses are knowingly selling lethal drugs. I am not talking about the fact that after years of research, study and testing to get approval to sell in the US, they know that X% of people taking this can and many will die. But if thast is your criteria for knowingly sell ing lethal drugs, we should remove all drugs from the marketplace, even antibiotics are lethal and we knowingly sell them.
